New Student Application Form

A $100 non-refundable application fee is due for each child at the time of application.

Receipt of both the New Student Application and the application fee are required for a child to be added to the school's overall enrollment/waitlist.

The Sweetgrass School's business hours are 7:15am- 5:30pm.

Parents & children are free to arrive to/leave from the building at any time during business hours.

Any child picked up after 5:31pm will be charged a “Late Pickup Fee.”

This is flat rate of $25, with an additional $25 every 10 minutes beginning at 5:40.

I understand that my child must be fully vaccinated, with an up-to-date SC immunization record on DHEC form. Religious exemptions are not permitted.

I understand that once enrolled, I will be required to maintain an ACH authorization form, linked to a checking or savings account, from which all bi-weekly tuition payments will be drafted.

After form submission, you will receive a follow-up email with instructions for paying the $100 application fee through www.MyProCare.com. Please allow up to 1 business week for a reply.
